The DeckLink 4K Extreme 12G features a mezzanine card design with 4K HDMI inputs and outputs on a daughterboard, along with the connections for connecting two optional modules for Dual Link SDI optical inputs and outputs. The card also supports AES/EBU balanced XLR and unbalanced RCA inputs, analog video I/O, a reference input, and RS-422 control via an included breakout cable.

The DeckLink 4K Extreme 12G can handle various compressed video formats such as ProRes and DNxHD, as well as 12-bit 4:4:4 compressed video. When working with uncompressed video, all images remain pixel-for-pixel identical to the original with no loss of image quality. This ensures sharper images when working with green screen, a cleaner composition and superior color correction. The card also supports common file formats for use in popular video editing, editing and design software. The DeckLink 4K Extreme 12G is compatible with Mac, Windows and Linux.
